Recording of Outcome Focused Care

This Recording Outcome Focused Care Training course is designed to support healthcare and social care staff in developing the knowledge and skills required to accurately document care that reflects individual outcomes, progress, and person-centred support.

​The training focuses on the importance of clear, factual, and professional record keeping, ensuring that care documentation demonstrates how support provided has positively impacted an individual’s wellbeing, independence, and agreed outcomes. Participants will explore best practice approaches to recording care, legal and professional responsibilities, confidentiality, and the importance of effective communication within care records.
